Simply select the self-directed plan you prefer from the options below. Before you begin, please have the following ready:

A valid government issued photo ID

Your beneficiary’s information (name, relationship, SSN, address, percentage designation)

Digital copy of a recent account statement from your financial institution (if transferring cash and/or assets)

If applicable, the Affiliate Code provided by the individual or company who referred you

Digital copy of a voided check (if setting up automatic transfers)
Once you have completed the online application, you will be prompted to print, sign and mail the following documents:

Account application acknowledgement page

Transfer or Conversion Form (if transferring funds from another financial institution)

Pre-Consent eSign Authorization Disclosure Form (to allow digital, quick signatures on all future paperwork processes)

Pay via Paypal or add credit card details to application form for processing
